Position Details

This is a field-based role with a home-based office, offering flexibility for those in the Southeast Houston, TX market to work a hybrid/field schedule. The role involves approximately 75% travel, dealing with in-home and facility-based environments.

Primary Responsibilities
  • Provide comprehensive quality care through communication with members in person or via phone
  • Support members with condition education, medication reviews, and resource connections
  • Assist with transitioning members from care facilities back to home
  • Work autonomously, managing complex issues and serving as a resource to others

Minimum Qualifications
  • Associate’s degree or higher

Compensation

The hourly salary range is $28.61 to $56.06, based on experience, location, and certifications. Benefits and incentives are included, subject to eligibility.
