In this Health and Social Services Coordinator role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, ensuring they have the right services to meet their needs.
Monday through Friday/No Weekends/No Holidays
If you reside within a commutable distance of Lowell, Lexington, MA and Concord, NH areas, you will have the flexibility to work remotely as you take on some tough challenges.
This is a Field-Based role with a Home-based office
Primary Responsibilities:
Expect to spend about 80% of your time in the field visiting our members in their homes or in long-term care facilities. You’ll need to be flexible, adaptable and, above all, patient in all types of situations.

The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
