Paradigm Home Health Companies seeks an experienced Registered Nurse to provide high-quality home health care in the Tyler area. You will deliver wound care, meds administration, IV therapy and patient education while maintaining thorough clinical documentation.

Applicants must hold a Texas RN license with at least 1 year of Medicare home health experience, CPR certification, and a valid TX driver’s license. Travel to multiple locations is required; WellSky EMR knowledge is a plus.

Qualifications

• Must hold a valid Texas RN license.
• At least 1 year of Medicare home health experience.
• CPR certification required.
• Willingness to travel to multiple locations.
• Knowledge of WellSky EMR software is preferred.

Responsabilités

• Provide clinical care including wound care, medication administration, disease education and IV therapy.
• Ensure documentation reflects medical necessity and quality patient care.
• Submit OASIS and other clinical documentation timely.
• Communicate patient needs with the interdisciplinary team.

• Provides exceptional clinical care that includes skills such as wound care, medication administration, disease process education, and IV therapy.
• Ensures clinical documentation demonstrates medical necessity and appropriately reflects quality patient care.
• Submits OASIS and other clinical documentation in a timely manner.
• Communicates patient's clinical needs with interdisciplinary team.

Qualifications & Skills

• Licensed Registered Nurse in the State of Texas
• 1+ years Medicare home health experience
• Valid Texas Driver's License and automobile insurance
• Travel to multiple locations
• Certification in CPR
• Knowledge of WellSky EMR software desired
