Qualifications

• Active RN license (TX/Compact) and CPR certification.
• Prior home health care experience with geriatric patients.
• Self-starter with strong organization and compassionate care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver skilled nursing care in patients' homes per physician orders and care plan.
• Collaborate with the care team to update personalized plans of care.
• Administer treatments and manage clinical activities for outcomes.
• Monitor progress and communicate findings to the team and family.
• Perform additional duties as assigned.

Essential Job Functions

• Deliver Skilled Nursing Care - Provide high-quality, comprehensive nursing care in the patient's home in accordance with the physician's orders and individualized plan of care.
• Collaborate on Plan of Care - Partner with the interdisciplinary care team to develop, implement, and update personalized care plans that address each patient's unique needs.
• Manage Treatments and Interventions - Administer nursing treatments and manage clinical activities to support optimal health outcomes for each patient.
• Monitor and Evaluate Progress - Continuously assess patient progress and the effectiveness of services; communicate findings with the care team, patient, family, physician, and case manager.
• Additional duties as needed.

Requirements

• Active RN License (TX/Compact)
• Active CPR Certificate
• Home Health Experience – Prior experience ingeriatric home health care.
• Self-Starter with Organizational Skill
