We currently seek an experienced Home Health Care Registered Nurse, preferably with Mediport, PICC Line, Wound Care, and Infusion experience.

The corporate office is located in Southfield, MI, and the individual will provide RN support throughout our service area within the Tri-County area (Oakland, Macomb & Wayne).

Responsibilities:

• Provide services according to the plan of care.
• Conduct initial evaluations and regular re-evaluations of patients’ nursing needs.
• Initiate and revise the plan of care as needed.
• Deliver skilled nursing services requiring specialized skills.
• Implement preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes for each patient visit and care conference summaries promptly.
• Coordinate services and inform personnel of patient condition changes.
• Counsel patients and families in meeting nursing and related needs.
• Participate in and present in-service programs.
• Adhere to agency policies and procedures.
• Process orders, notify physicians of patient needs, and complete certification/recertification and discharge summaries.
• Assess and determine nursing needs for each patient.
• Refer patients to specialized therapists as needed.
• Supervise and educate other nursing personnel.
• Conduct patient care conferences and participate in peer review and quality improvement activities.
• Participate in utilization review.
• Provide total patient care as needed.
• Take on-call duties during nights, weekends, and holidays.
• Complete and submit OASIS assessments and related documentation timely.
• Utilize ICD-10 codes appropriately.

Working Environment:

Work indoors at the agency office and in patient homes, with travel within the service area.

Qualifications:

• Graduate of an approved nursing program and current Michigan license.
• At least one year of nursing experience.
• Kinnser EMR experience preferred.
• Home healthcare experience preferred.
• Experience with Wound Vac, PICC line, and Mediport care preferred.
• Pediatric experience is a plus.
