Become part of the Hope At Home family and apply today! We currently have an opening for an experienced Home Health Care Registered Nurse, preferably with PICC Line, Wound Care, and Infusion experience.

The corporate office is located in Southfield, MI, and the nurse will provide support throughout the Tri-County area (Oakland, Macomb, Wayne).

A Registered Nurse administers skilled nursing care to patients in their homes, following physician's orders and care plans under supervision.

Qualifications

• Graduate of an approved nursing program and licensed in Michigan
• At least one year of nursing experience
• Kinnser EMR experience preferred
• Home Healthcare experience preferred
• Experience with wound vac, PICC line, and mediport care preferred
• Pediatric experience is a plus

Responsibilities

• Provide services according to the care plan
• Conduct initial and ongoing patient evaluations
• Develop and revise care plans as needed
• Perform specialized nursing procedures
• Prepare and maintain clinical documentation
• Coordinate services and communicate patient needs
• Educate patients and families
• Participate in in-service training and quality improvement activities
• Follow agency policies and procedures
• Process orders, notify physicians of changes, and complete documentation
• Refer patients to specialists when necessary
• Supervise and teach other nursing staff
• Participate in patient care conferences and peer reviews
• Provide total patient care as needed
• Take on-call duties during nights, weekends, and holidays
• Complete required assessments and documentation within specified timeframes
• Utilize ICD-10 coding appropriately

Work Environment

Work indoors at the agency office and in patient homes, with travel within the service area.
