Job Description
Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.
Job Responsibilities
- Assesses the patient's physiologic health status
- Conducts patient interview, explains policies and procedures, reviews patient's chart, and answers questions correctly and courteously;
- Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health status;
- Determines mobility, sensory deficits, prostheses use, and skin condition, and adjusts the plan of care accordingly;
- Assesses pain level and pain management;
- Communicates and documents patient's physiologic health status and plan of care.
Assesses patient's psychosocial health status- Gathers perception of medical/nursing care and patient expectations;
- Determines coping mechanisms, knowledge level, and comprehension ability;
- Identifies cultural/ethnic requirements to reduce anxiety and ensure satisfaction;
- Documents psychological status and care plan; includes support groups, counseling, and social services.
Identifies patient outcomes- Develops criteria for measuring outcomes;
- Identifies actual and potential patient problems;
- Determines patient's need for teaching based on psychosocial and developmental assessments;
- Creates patient outcome statements and establishes individualized goals.
Formulates the plan of care and sets priorities- Identifies care activities and prioritizes to achieve expected outcomes;
- Coordinates the use of supplies, equipment, and medication efficiently;
- Documents the plan and collaborates with the healthcare team.
Implements the plan of care- Maintains vigilance to ensure safety precautions are followed;
- Applies professional skills related to the plan;
- Reassesses and revises the plan as needed;
- Administers treatments correctly;
- Uses equipment properly;
- Provides emotional support;
- Performs procedures safely and efficiently;
- Records observations and responses accurately;
- Participates in discharge planning and provides instructions.
Evaluates care effectiveness- Measures and documents outcomes;
- Performs variance analysis for quality improvement;
- Implements systems to improve care;
Collaborates with the care team- Works with physicians on care plans;
- Interprets and implements treatment regimens;
- Assists during treatments and procedures;
- Communicates changes in patient status;
- Delegates appropriately to team members.
Performs grade I-IV Decubitus Care- Prevents skin breakdown;
- Applies dressings and compresses;
- Performs irrigations and related procedures.
Administers medications safely- Identifies medications and understands their effects;
- Prepares and administers medications correctly;
- Educates patients and families about medications.
Participates in patient and family education- Explains conditions and care;
- Records and communicates assessment data;
- Utilizes teaching resources;
- Plans for discharge and referrals;
- Evaluates teaching effectiveness.
Performs related duties as requiredJob Qualifications
- Graduate from an accredited School of Nursing; Bachelor's degree preferred. Enrolled in an accredited program within 24 months if hired after Jan 1, 2013, and obtain a BSN within 5 years.
- Current NY State RN license.
- BLS, ACLS certifications required.
- Documented Oncology experience required.
