Job Description

Performs an age-specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating patient care and delegating activities to the patient care team.

Job Responsibilities
  1. Assesses the patient's physiologic health status
    • Conducts patient interviews, explains policies and procedures, reviews charts, and responds courteously to questions;
    • Assesses gastrointestinal, cardiovascular, respiratory, renal, and neurological health;
    • Determines mobility, sensory deficits, prostheses use, and skin condition, adjusting care plans accordingly;
    • Assesses pain levels and management needs;
    • Documents and communicates the patient's physiologic health status and care plan.
  2. Assesses patient's psychosocial health status
    • Gathers perceptions of care and expectations;
    • Determines coping mechanisms, knowledge, and comprehension abilities;
    • Identifies cultural and ethnic requirements to reduce anxiety and improve satisfaction;
    • Documents psychological status and care plans, including support groups and counseling services.
  3. Identifies patient outcomes
    • Develops criteria for measuring outcomes;
    • Identifies actual and potential problems;
    • Determines teaching needs based on psychosocial and developmental assessments;
    • Establishes patient outcome statements and individualized goals.
  4. Formulates the plan of care
    • Sets priorities and care activities to meet expected outcomes;
    • Coordinates resources effectively;
    • Documents and collaborates with the healthcare team.
  5. Implements the plan of care
    • Maintains safety and follows precautions;
    • Administers treatments and uses equipment correctly;
    • Provides emotional support;
    • Reassesses and revises care as needed;
    • Documents care and patient responses accurately;
    • Acts swiftly during emergencies;
    • Participates in discharge planning and provides instructions.
  6. Evaluates care outcomes
    • Measures effectiveness and documents outcomes;
    • Performs variance analysis for quality improvement;
    • Implements systems to enhance care quality.
  7. Collaborates with the care team
    • Works with physicians and delegates appropriately;
    • Assists during treatments and diagnostic procedures;
    • Reports patient condition changes timely.
  8. Performs grade I-IV Decubitus Care
    • Performs preventive skin care and dressing applications;
    • Performs irrigations and other skin procedures.
  9. Administers medications safely
    • Identifies medications and educates patients;
    • Prepares and administers medications, including IVs;
    • Documents administration and educates on drug interactions.
  10. Participates in patient and family education
    • Explains conditions and care plans;
    • Records and updates assessment data;
    • Utilizes teaching resources;
    • Plans for discharge and post-hospital care;
    • Evaluates teaching effectiveness.
  11. Performs other related duties as required.

Job Qualifications
  • Graduate of an accredited School of Nursing; Bachelor’s preferred; must enroll in an accredited program within 24 months if hired after Jan 1, 2013, and obtain a BSN within five years;
  • Current New York State RN license;
  • BCLS and ACLS certification required.
