RN, Evergreen Assisted Living (Part Time)
Location: Woonsocket, RI

Part-time (8-10 hours/week), preferably two 4–5-hour days per week, but negotiable.

Evergreen Assisted Living in Woonsocket, RI, seeks a compassionate, professional Registered Nurse.

Evergreen Assisted Living is a 26-bed F2, M1 assisted living residence in Woonsocket, RI, specializing in serving adults with mental illness.

Must work collaboratively with CNA/Med Techs, caseworkers, community providers, the agency Director of Nurses, and the Administrator to care for adults with behavioral health disorders.

Responsibilities:

  • Assess applicants for admission to Evergreen in collaboration with the Administrator.
  • Update Appendix C resident assessments annually.
  • Assess residents monthly for health changes, smoking status, proper self-administration of injectable meds, and use of CPAP.
  • Verify the accuracy of MARs and medication orders with medical providers.
  • Evaluate CNA/Medication Aides quarterly for skill development.
  • Coordinate pharmacy services for medication fulfillment.
  • Perform medication reconciliation and communicate with providers, including VA, PCPs, and specialists.
  • Maintain a flexible schedule to check scheduled monthly med deliveries and meet regularly with clients and staff.
