Qualifications

  • Hold a current RN license in Massachusetts and meet related specialty credentials.

  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and interdisciplinary utilization.
  • Meet National Benchmarks for annual outcomes.
  • Refer to community resources as appropriate.
  • Evaluate patient’s clinical needs; develop, implement, and revise the plan of care; address all problems and document accordingly.
  • Participate in interdisciplinary patient case conferences; evaluate care plan effectiveness.
  • Supervise and instruct Home Health Aides’ plans of care every 14 days with documentation.
  • Notify MD/other care providers of significant patient condition changes.
  • Complete admission and other paperwork promptly.
  • Provide clear, concise narrative documentation reflecting skilled care, teaching, and patient/caregiver responses.
  • Obtain and clarify valid MD orders for all care.
  • Plan and coordinate discharges; inform patient/MD; record discharge disposition and complete all discharge paperwork.
  • Perform all other duties as assigned.
  • Pass the VNA pre-hire medication test successfully.
  • Hold a current RN license in Massachusetts and meet related specialty credentials.
  • Minimum of 1 year nursing experience or 6 months with a BSN.
  • CPR certification required.
  • Home care, IV experience, computer skills, and a BSN preferred.

Purpose of Position

Conduct comprehensive nursing assessments upon patient admission, considering physiological, psychological, environmental factors, and health history to ensure safe, quality care. Integrate assessment data, identify problems, and develop a case management care plan.

Schedule Details

Baylor schedule, 30 hrs/week (paid for 37.5), 3p-11p, with 30-minute unpaid lunch. Rotating evenings, weekends, and holidays. Sign-On Bonus Eligible!
