Job summary

Knox Community Hospital in Mount Vernon, OH is seeking a Registered Nurse for on-call rehab coverage. You will assess patients, revise care plans with the rehab team, and deliver safe, prioritized nursing interventions to support discharge planning.

Responsibilities include evaluating patient responses, coordinating with physicians and colleagues, delegating tasks when needed, and educating patients and families on treatments and post-hospital care.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current licensure to practice professional nursing in Ohio.
• Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
• Specialty certification preferred; ACLS and PALS required within 1 year of hire.

Responsibilities

• Develops/revises nursing plans of care with rehab team to address deficits and discharge needs.
• Delivers safe rehabilitation nursing interventions in an organized, efficient, and prioritized manner.
• Evaluates patient response to interventions and progress toward discharge goals.
• Collaborates with physicians and healthcare providers to coordinate care plans.
• Accepts delegated responsibilities such as charge nurse on assigned shift.
• Delegates responsibilities to appropriate personnel.
• Provides patients and families with education regarding treatments and pre/post hospital care.

On Call Requirements: 1/ week and every 4th weekend

PRIMARY JOB RESPONSIBILITIES

• Assesses/reassesses patient’s condition based upon observation of patient and interview of patient and/or family, physician’s diagnosis and data, diagnostic results, and relevant data from other healthcare providers. Assessment and reassessment of functional and cognitive deficits as it relates to transition to the community.
• Uses professional knowledge to develop/revise nursing plans of care for assigned patients according to unit guidelines and criteria with expected outcomes. Works with rehab team members to construct, develop, and implement rehabilitation treatment plan and rehabilitation plan of care to address emergent medical issues as well as functional/self-care/cognition deficits.
• Delivers safe rehabilitation nursing interventions in an organized, efficient, and prioritized manner to address identified needs.
• Evaluates/re-evaluates patient’s response to rehab nursing interventions and patient’s progress to expected outcome(s) and community discharges.
• Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical and nursing plans of care.
• Accepts delegated responsibilities for functions such as, but not limited to, charge nurse for unit activities on assigned shift.
• Delegates responsibilities to appropriate personnel.
• Provides patients and family members with relevant health care education and instructions regarding treatments and procedures including pre/post hospital care as appropriate.

Requirements Include

• Graduate of an accredited program for nursing education, BSN preferred.
• Current licensure to practice professional nursing in the State of Ohio.
• Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
