Qualifications

• Critical thinking and clinical judgment required to assess patient data and develop a care plan.
• Effective communication with patients, families and interdisciplinary teams is essential.
• Ability to participate in discharge planning and coordinate care transitions.
• Delegation and supervision of nursing staff where appropriate.
• Adherence to hospital policies and safety standards is mandatory.
• CPR/BLS and other required certifications as per AH policy.

Responsibilities

• Delivers coordinated nursing care for a patient or assigned group of patients following standards and nursing process.
• Supervises and directs activities of nursing staff; coordinates care with other disciplines.
• Utilizes critical thinking to develop and implement care plans; evaluates patient progress.
• Participates in discharge planning to ensure continuity of care.
• Collaborates with patient, family and healthcare team; provides health information and education.
• Performs other job-related duties as assigned.
