Responsibilities

• Performs daily rounds on patients and coordinates care to ensure quality service.
• Develops a plan of care reflecting patient needs across multiple domains.
• Assesses and plans nursing interventions, including discharge planning.
• Monitors and responds to patient pain per procedures.
• Identifies early warning signs and escalates to clinicians as needed.
• Assists with emergency measures and documents observations.
• Communicates with the care team and ensures timely referrals.
• Completes incident reports and provides patient education for discharge.
