St. David's South Austin Medical Center is seeking an experienced Registered Nurse for the Admission and Discharge role. You will assess patients, coordinate admissions, ensure accurate discharge documentation, and educate families on treatment plans and follow-up steps.

You will advocate for patients, work with physicians and care teams, and help create a seamless transition from hospital to next-stage care, supporting optimal outcomes and patient satisfaction.

Qualifications

• RN licensure as required by state
• BLS certification must be obtained within 30 days of start
• Associate or Bachelor’s degree in nursing or nursing science

Responsibilities

• Conduct nursing assessments for admission and discharge
• Assist with risk assessments, medication reconciliation, and discharge documentation
• Educate patients, families and caregivers on conditions, treatment plans and follow-up care
• Complete discharge education and ensure patients receive copies of paperwork and referrals
• Coordinate with nursing leadership to prioritize workload and improve patient transitions
• Prioritize tasks and manage time to support smooth inpatient flow
