Job summary

HCA Healthcare, Inc. is seeking a Registered Nurse Case Manager at St. David's South Austin Medical Center to support patients and families across the care journey.

You will coordinate services, connect resources, and develop care plans in collaboration with physicians, nurses, and department leaders to ensure safe transitions and consistent quality. You will guide program orientation, discharge criteria, and treatment planning, while documenting psychosocial assessments and communicating needs

Qualifications

• Registered Nurse licensed to practice in Texas.
• Minimum 5 years of experience in case management or related nursing roles.
• Experience coordinating care across disciplines and communicating with patients and families.

Responsibilities

• Guide patients and families through program orientation and discharge planning.
• Coordinate education for patients, families, and caregivers to support treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs.
• Complete psychosocial assessments and discharge planning evaluations with clear documentation.
• Collaborate with Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care.
• Promote the health system’s values through every patient and family interaction.
