Job Summary
The RN Digital Health is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families, and significant others receive individualized high-quality, safe patient care. They practice remotely in a non-traditional clinical setting where virtual health consults are performed using video, mobile, telephonic, and remote patient monitoring technologies. The virtual clinic is administered by the Executive Director of Digital Health (RN)/Medical Director (MD) and supported through the WellStar Clinical Champion Forum. Practice is guided by the WellStar Professional Practice Model, 5-Star Nursing, evidence-based practice, and research. RN competencies are derived from these models and supported by the WellStar Core Competencies.

Core Responsibilities and Essential Functions
- Perform nursing processes including assessment, diagnosis, outcome identification, planning, implementation, and evaluation using evidence-based practice and critical thinking to ensure individualized care and optimal patient outcomes.
- Provide relationship-based, patient-centered care tailored to population-specific characteristics, respecting cultural diversity.
- Partner with patients and significant others through effective communication methods such as bedside reports, interdisciplinary rounds, patient teaching, and updates, focusing on patient goals.
- Practice according to current clinical standards, promoting teamwork, interdisciplinary communication, timely documentation, and a respectful, inclusive environment.
- Demonstrate teamwork and collaboration, practicing in a fiscally responsible manner that supports healthcare team recruitment and retention.
- Participate in performance improvement activities, research, and clinical champion initiatives to promote safe, quality care.
- Complete all professional competency assessments and mandatory education; serve as a preceptor or mentor to foster workforce development.
- Promote evidence-based practice, nursing research, and performance improvement, including participation in value analysis and protocol development.
- Adhere to healthcare policies, legal, regulatory, and accreditation standards, including TJC Patient Safety Goals and Wellstar safety absolutes.
- Participate in data collection, address clinical questions with evidence, and utilize appropriate resources.
- Proactively plan patient care across the continuum, supporting protocols to improve outcomes like readmission rates.
- Support efficient resource use and enhance patient experience through operational excellence and follow-up.
Minimum Education
- Associate's Degree from an accredited nursing school (required)
- Bachelor's Degree in Nursing (preferred)
Licenses and Certifications
- RN license (Multi-state Compact or Single State)
- Basic Life Support (BLS) certification
Experience
- Minimum 5 years RN experience in various outpatient or community settings, including adult and pediatric care (required)
- Minimum 1 year experience in virtual care services (preferred)
Skills
- Proficiency in English, technology use, and basic computer skills
- Strong interpersonal, collaboration, and customer service skills
- Ability to work in a fast-paced environment, respond to emergencies, and demonstrate critical thinking and time management skills
