Qualifications

• Active NY RN license is required.

Responsibilities

• Conducts ongoing health assessments and supports individuals' health needs in residential, day habilitation and respite settings.
• Provides clinical response and guidance for health concerns and medical emergencies.
• Coordinates medical care with physicians, specialists, pharmacies, and medical liaisons; supports person-centered care planning.
• Facilitates timely scheduling and follow-up on medical recommendations with the Medical Liaison.
• Develops and maintains Plans of Nursing Service outlining care and delegated DSP responsibilities.
• Oversees medication management including MAR review and reconciliation.
