Affinity is looking for reliable, compassionate, professional Registered Nurses (RNs) to care for our patients in Delaware County. The ideal RN professional is highly skilled, organized, and compassionate. We have a variety of opportunities available (e.g., Shift Care, Per Diem, Start of Care (SOC), Skilled Visits, Hourly assignments, etc.) depending on your availability and preferences. We use Axxess and HHAexchange.

Responsibilities

• Record and assess a patient’s medical history accurately
• Monitor and record vital signs including blood pressure, temperature, heart rate, etc.
• Perform Start of Care (SOC) visits and ongoing patient evaluations
• Develop and update patient care plans in coordination with physicians
• Monitor patients’ condition and identify progress or side effects of treatments
• Administer medications, treatments, and IV therapy as prescribed
• Educate patients and families and provide emotional support
• Maintain accurate documentation and ensure compliance with home health standards

Skills

• Proven experience as a Registered Nurse
• Ability to manage patient care independently and within a team
• Familiarity with home health care standards and procedures
