OverviewTo provide care coordination across the care continuum from inpatient to outpatient settings while assessing needs and optimizing adherence by identifying and overcoming barriers such as language, cognitive, cultural, social, financial, environmental, psychological, motivation, and personality disorders, along with overall chronic illness management. Mobilizes appropriate community referrals and resources as indicated and provides detailed, recurrent patient and family education. Involved in program development for both inpatient and outpatient settings and with interpretation of program metrics.

ResponsibilitiesClinical Excellence and Coordination
- Coordinate diagnostic, therapeutic treatment plans, and educational efforts for inpatient/outpatient patients in collaboration with the Heart Failure/Pulmonary Hypertension Medical Director and interdisciplinary team.
- Facilitate appropriate referrals to the program by providing and coordinating care and communication with referring physicians and offices.
- Conduct regular follow-ups during care transitions with patients, families, coordinators, and other health professionals.
- Deliver high-quality care utilizing the nursing process, demonstrating knowledge and clinical skills per policies, procedures, and standards.
- Perform advanced medication reconciliation in complex, changing regimens.
- Adjust diuretics and Beta Blockers using physician-developed and approved protocols.
- Coordinate care with drug manufacturers and pharmacies to obtain medications and patient assistance.
- Assess and optimize home medication delivery systems.
- Set periodic RN-initiated phone surveillance contacts.
- Enhance adherence by identifying and overcoming barriers such as language, cognitive, cultural, motivational, personality, and health literacy issues.
- Cultivate support systems to improve outcomes.
- Participate in the evaluation and care of hospitalized patients and those receiving outpatient infusion therapy.
- Provide emotional and psychological support for patients and families with advanced chronic illness.
- Follow up clinically and laboratorily after initiation of therapies.
- Ensure timely documentation in the electronic health record following standardized workflows.
Development and Education
- Provide detailed, recurrent education to patients and families on home health surveillance, diet, and fluid restrictions.
- Identify and work to resolve barriers to adherence.
- Develop the Heart Failure/Pulmonary Hypertension Program for inpatient and outpatient settings, including staff education on assessment, referral, and medication protocols.
- Educate about medication management and actions for complications.
- Support and teach regarding diagnosis, disease process, and discharge planning.
- Conduct educational sessions, including didactic conferences, clinical discussions, and web-based learning.
- Obtain advanced certification in the practice area (e.g., AAHFN) if encouraged.
- Participate in or assist with research studies related to Heart Failure/Pulmonary Hypertension.
- Engage in professional development and share knowledge through networking and research.
- Participate in committees and program development work related to Heart Failure/Pulmonary Hypertension.

- Demonstrate clinical leadership as a role model and provide direction to ensure top-of-licensure care.
- Utilize critical thinking for independent judgment in patient care decisions.
- Engage in process improvement and quality initiatives to ensure high-quality multidisciplinary care.
Qualifications- Graduate of an accredited nursing program.
- Minimum of 1-2 years experience in a clinical cardiac setting (inpatient or outpatient).
- Mandatory Reporter Certification.
- BLS Certification.
- Fluent in English (reading, writing, speaking).
- Multicultural sensitivity.
- Basic computer skills with Microsoft Office.
- Customer/patient focused.
- Critical thinking skills and independent judgment.
- Ability to use usual and customary equipment for the position.
