The Transplant Coordinator is responsible for coordinating and managing the care of transplant patients while waiting for a transplant. This role provides clinical expertise, patient education, care coordination, regulatory compliance, and interdisciplinary collaboration to support kidney, kidney-pancreas, and pancreas transplant programs.

• Assess patient health status
• Serve as a communication link between physicians, patients, families and multidisciplinary team members.
• Maintain compliance with United Network for Organ Sharing (UNOS), OPTN, CMS, and organizational requirements.
• Complete transplant-related documentation, including patient listings and status changes.
• Collaborate with case management, financial coordinators, social workers to support patient needs.
• Maintain accurate and current patient records and transplant databases.

Qualifications

Required

• Current Registered Nurse (RN) license in the State of Louisiana.
• Bachelor of Science in Nursing (BSN) or higher.
• Minimum of three years of clinical nursing experience.
• Strong assessment, communication, organizational, and critical thinking skills.
• Ability to work collaboratively in a multidisciplinary healthcare environment.

Preferred

• Certification in transplant nursing (CCTC, CPTC, or equivalent).
• Experience with UNOS/OPTN regulations and reporting requirements.
