Job summary

Platinum Communities in Wisconsin seeks a Registered Nurse to provide clinical oversight and direct nursing services across Community-Based Residential Facilities (CBRFs) and RCACs. You will travel between communities to support multiple facilities and work with leadership to ensure high-quality resident care and regulatory compliance.

Description

The Registered Nurse (RN) provides clinical oversight and nursing services to residents within the company’s Community-Based Residential Facilities (CBRFs) and Residential Care Apartment Complex (RCACs) in accordance with Wisconsin DHS regulations. This role supports multiple Platinum Communities as needed and requires travel between communities. The RN works collaboratively with leadership and care teams to ensure quality resident care, regulatory compliance, and staff support.

Requirements
Clinical Oversight & Resident Care
  • Perform comprehensive nursing assessments and ongoing evaluations of residents.
  • Develop, review, and update resident care plans, service plans, and health-related documentation.
  • Coordinate and oversee medication management, including medication reviews and physician communication.
  • Monitor residents’ health status and respond to changes in condition appropriately.
  • Provide nursing interventions within RN scope of practice and regulations.
Regulatory Compliance
  • Ensure compliance with Wisconsin DHS regulations and Platinum Communities policies.
  • Assist with audits, surveys, inspections, and plan of correction development as needed.
  • Ensure proper documentation and timely completion of required assessments and reports.
Staff Support & Education
  • Provide clinical guidance, training, and support to administrators, med techs, and caregivers.
  • Assist with competency evaluations, medication administration training and delegation, and ongoing education.
  • Serve as a clinical resource for staff regarding resident care and best practices.
Care Coordination
  • Collaborate with physicians, pharmacies, hospices, home health agencies, and other healthcare providers.
  • Participate in care conferences as required.
  • Assist with admissions, discharges, and transitions of care.
Multi-Community Support
  • Provide RN coverage and support at other Platinum Communities as assigned.
  • Travel between communities to meet operational and clinical needs.
  • Support leadership with clinical initiatives, quality improvement efforts, and special projects.
Qualifications:
Required
  • Current, unrestricted Registered Nurse (RN) license in the State of Wisconsin.
  • Knowledge of Wisconsin CBRF and RCAC regulations and DHS requirements (or willingness to learn).
  • Strong clinical assessment, documentation, and communication skills.
  • Ability to work independently and manage time across multiple locations.
  • Valid driver’s license and reliable transportation; willingness to travel between communities.
  • Occasional on-call or flexible scheduling may be required based on community needs.
Preferred:
  • Experience in assisted living, long-term care, or geriatric nursing.
  • Prior experience providing clinical oversight across multiple facilities.
  • Familiarity with electronic health records and care management systems.
