Qualifications

Responsibilities

• Assess patient condition during admission and each shift; report changes.
• Perform procedures and document care in the medical record promptly.
• Respond promptly to patient requests and anticipate needs.
• Coordinate care with a multidisciplinary team to deliver a holistic plan.
• Interface between healthcare providers and community supports for ongoing care.
