Job summary

Era Living seeks a Community Health Director in Bellevue, WA, to lead health and wellness services for residents and supervise a compassionate healthcare team. The role requires RN licensure in WA, 5+ years of nursing experience with leadership, and strong program development and regulatory knowledge.

Qualifications

• Experience supervising, directing and evaluating direct care services in a Wellness Center.
• Ability to design Wellness Plans and complete pre Move-In assessments for residents.
• Review of Advance Directives (POLST).
• Knowledge of Washington Boarding Home regulations.
• Budget management including staffing and supplies.
• Assist residents with ADLs or medications as needed.
• Provide educational programs for residents community-wide.
• Document resident status and plan of service in writing.
• Strong English reading/writing and communication skills.
• Pass DSHS background check.

Responsibilities

• Oversee overall management of resident Health and Wellness services.
• Lead a healthcare staff team and provide RN delegation as required.
• Develop and implement health education and health promotion activities for older adults.
• Ensure accurate documentation of resident status and service plans.
• Collaborate with nursing staff to supervise Wellness Center and Personal Care services.
