Qualifications

• Active Registered Nurse license in Pennsylvania (BSN preferred).
• Minimum 2 years of clinical experience (emergency, critical care, acute care, or EMS preferred).
• Strong clinical judgment, communication skills, and ability to work independently in dynamic field environments.
• Passion for innovative, patient-centered care beyond the traditional hospital system.

Responsibilities

• Deliver pre-scheduled visits and acute-response care in patients' homes as primary provider in the field.
• Perform advanced nursing assessments, interventions, and follow-up under physician-directed protocols.
• Stabilize patients, prevent unnecessary readmissions, and support recovery.
• Collaborate with physicians, advanced providers, and EMS for seamless care transitions.
• Document care in electronic medical records and coordinate with the broader healthcare team.
• Serve as a clinical leader bridging hospitals, EMS, and primary care.
