Job summary

Mitchell Martin Inc. is seeking a Registered Nurse to join a mobile ACT team serving individuals with serious mental illness in community settings in Manhattan, NY. The role operates Mon-Fri, 9:00am-5:00pm, with compensation of $50-$55 per hour.

The RN will perform comprehensive nursing care, health assessments, medication management, and collaboration with primary care and community providers to support housing and resources for clients.

Qualifications

• Active RN license in New York State.
• Ability to conduct health assessments and coordinate care.
• Experience in community-based nursing is preferred.

Responsibilities

• Conduct physical and health assessments.
• Monitor ongoing health status and coordinate care.
• Administer IM injections and manage medications as prescribed.
• Provide psychiatric and substance use-related nursing support.
• Deliver health education, daily living skills training, and medication management.
• Coordinate medical treatment with primary care and community providers.
• Support individuals in obtaining stable housing and community resources.
