UW Health in Madison, Wisconsin seeks a Registered Nurse to join the Adult Sickle Cell Disease Program as an RN Clinical Program Coordinator. You will partner with a multidisciplinary team to deliver patient-centered care to adults with sickle cell disease, coordinating care plans and advocating for patients and families.

The role emphasizes health equity, clinical excellence, and community outreach, with 90% FTE, four 9-hour shifts weekly, and day shifts.

Qualifications

• BSN preferred with valid RN license in Wisconsin.
• Minimum 2 years nursing experience.
• Experience in teaching patients and families.

Responsibilities

• Collaborate with a multidisciplinary team to provide high-quality, compassionate care to adults with sickle cell disease.
• Establish goals and strategies for meeting ongoing patient needs.
• Provide care coordination and follow-up on care plans focusing on symptom management.
• Build trusting relationships and advocate for patients and families.
• Help patients overcome barriers to care and connect with resources.
• Support program development through data collection, quality improvement, and outcomes evaluation.
• Incorporate evidence-based practices and emerging advances into patient care.
• Participate in community outreach to expand access and reduce stigma.
