Responsibilities

• Participates in patient and client engagement through care planning and excellent customer service.
• Provides and ensures client care coordination and transition management activities across the continuum.
• Supports onboarding of new caregivers through clinical review of onboarding materials, interviews, competency, etc.
• Collaborates with scheduling function to review skills of caregivers and patient skill needs to promote safe, compliant staffing.
• Collaborates with the clinical team to perform documentation review.
• Provides education to team members, patients, family members of patients, caregivers, and referral sources.
• Conducts in-home assessments and investigations related to patient care, caregiver concerns, service delivery, and quality issues; documents findings and collaborates with qualified clinical leadership regarding follow-up actions and resolution.
• Provides clinical support and guidance to the care team regarding home care case management, care coordination, and clinical processes.
• Participates in clinical quality initiative activities.
• Partners with other job families to support office operations.
• Rotates on-call responsibilities with other clinical team members per a schedule established by the office.
• Accountable to all federal, state, local contracts and programs, and Maxim requirements.
• Performs other duties as assigned.

Position Description:

The Clinical Operations Associate is a development-focused role that builds foundational skills in clinical operations and homecare delivery. Under the direction of the Clinical Manager or other clinical leader, this position supports clinical coordination from admissions through discharge for homecare clients and patients served, gaining exposure to care management processes, regulatory requirements, and organizational standards.

The Clinical Operations Associate strengthens competency in compliance expectations across federal, state, and local programs, as well as internal requirements. This role also assists with caregiver orientation, evaluation, and competency assurance, while contributing to caregiver education and training initiatives and gradually building clinical judgment, operational knowledge, and leadership capability.

Minimum Requirements:

• One year of nursing experience required (RN experience preferred).
• Active RN license required (states in which office and patients are located).
• Must meet all federal, state, local contract and program requirements, in addition to internal certifications and training, as required.
• Ability to drive in order to travel frequently within assigned area to conduct clinical oversight and assessment of patient care and to support caregiver training and development.
• Excellent communication skills.
• Demonstrated critical thinking skills preferred.
• Supervisory experience preferred.Experience in quality management preferred.
• Computer proficiency including Microsoft Office Suite (Word, Excel, Teams, etc).
• Technological proficiency including mobile devices for documentation.
• Proficiency in the English language is required.

Compliance & Ethics Expectations:

• Participates and successfully completes the company’s compliance program requirements and adheres to the Code of Conduct, Company policies, and applicable federal and state requirements
• Sets an example for other employees regarding how the Company’s Code of Conduct and Compliance Program is applied and observed every day when dealing with customers, business operations, or other team members
• Reports potential violations of company policy, Code of Conduct, and/or applicable laws and regulations to the company hotline, through management to the Compliance and Ethics Department, or through other channels made available by the company for reporting potential violations
• Promotes an environment in which other employees are encouraged to report potential violations
• As appropriate, provides input and suggestions regarding areas in which policies, procedures, workflows, and/or controls can be improved to enhance compliance

Compensation: $95,000 + $5,000 Sign-On Bonus!
