Qualifikationen

• Strong organizational, planning and leadership skills.

Aufgaben

• Provide direct clinical supervision and performance evaluations for assigned staff.
• Conduct in-home supervisory visits with team members.
• Monitor caseloads and team productivity; optimize resource use for outcomes.
• Coordinate interdisciplinary care and patient care plans.
• Develop patient tracking and staffing reports for leadership.
• Report census and productivity to Administrator; identify staffing needs.
