Job summary

Life’s At Home Care is seeking a Virginia-licensed RN to lead clinical care in a home-based setting as RN Clinical Manager/Director of Nursing. You will perform initial assessments, develop care plans, monitor risks, coach caregivers, and collaborate with branch leadership to ensure safe starts and continuity of care.

The role blends independent field visits with clinical leadership, cross-branch coordination, and ongoing caregiver support.

Qualifications

• Current, unrestricted Virginia Registered Nurse license is required.
• At least one year of RN experience preferred; home care or related fields are considered.
• Clinical leadership or supervisory experience strongly preferred; prior DON title not required.
• Valid driver’s license, automobile insurance, reliable transportation, and ability to travel to clients’ homes are required.

Responsibilities

• Lead clinical assessment and care planning: perform initial assessments and required reassessments; develop individualized plans of care and caregiver instructions.
• Monitor clinical risks and manage care, including falls and hospitalization-related changes; determine reassessment or escalation.
• Coordinate with clients, families, physicians, VA contacts, home health, hospice and other care partners.
• Coach caregivers, identify competency gaps, and lead quarterly clinical training.
• Partner with Branch Manager on QA/QI reviews and follow-up actions to improve care quality and continuity.
• Support new client starts and retention, ensuring safe admissions and addressing care-related concerns.
• Maintain clinical documentation in electronic records and participate in weekly RN/Branch Manager coordination.
• Assist with admission paperwork when assigned.

The Opportunity

We are seeking a Virginia-licensed RN who wants to lead clinical care in a home care setting—not simply complete visits. As our RN Clinical Manager / Director of Nursing, you will complete initial client assessments and required reassessments, develop and update individualized care plans, monitor clinical risks, coach caregivers, and partner with branch leadership to strengthen quality, safe client starts, and continuity of care. This position combines independent field visits with collaborative clinical leadership.

What You Will Lead

• Clinical assessment and care planning: Complete initial assessments and required reassessments in clients’ homes; create, review, and update individualized plans of care and clear caregiver instructions.
• Care management and risk response: Monitor significant changes in condition, falls, hospitalization-related changes, mobility and transfer concerns, and other reported clinical risks; determine appropriate reassessment, escalation, or referral within the agency’s scope.
• Care coordination: Communicate clinically relevant needs and changes with clients, families, physicians, VA contacts, home health, hospice, and other authorized care partners, as appropriate.
• Caregiver clinical support: Educate and coach caregivers, identify competency gaps, validate relevant skills, and help lead quarterly clinical training.
• Quality assurance and improvement: Partner with the Branch Manager to lead quarterly QA/QI reviews, examine clinical trends and documentation findings, assign corrective actions, and verify follow-through.
• New-client starts and retention: Work with the Client Care Coordinator and Branch Manager to support timely, safe admissions and resolve care-related concerns that may affect client satisfaction or service continuity.
• Clinical documentation and collaboration: Maintain timely records in the agency’s electronic systems; participate in a structured weekly RN/Branch Manager coordination meeting and communicate urgent risks promptly.
• Admission backup: When assigned, assist with admission paperwork in addition to clinical assessments and plans of care.

Qualifications

• Current, unrestricted Virginia Registered Nurse license required.
• At least one year of relevant RN experience preferred; home care, home health, hospice, private duty, hospital, or related experience considered.
• Clinical leadership, care management, caregiver training, or supervisory experience strongly preferred; prior DON title not required.
• Sound assessment and care-planning judgment; confident communication, organization, documentation, and collaborative problem-solving skills.
• Valid driver’s license, automobile insurance, reliable transportation, and ability to travel to clients’ homes required.

Compensation & Benefits

• $88,000–$91,000 annual salary, based on qualifications and experience.
• Performance-based bonus opportunity, subject to the agency’s bonus plan.

• 3% employer retirement match, subject to plan eligibility and terms.

Work Arrangement

Full-time role supporting the Chesapeake branch, with regular travel to client homes and collaboration with office leadership. Routine after-hours on-call coverage is handled by a separate team member. The RN may be contacted occasionally to provide clinical guidance or escalation support for urgent clinical concerns outside normal business hours. These situations are expected to be infrequent; the RN is not responsible for routine scheduling or staffing calls.
