We are currently seeking a dedicated and experienced RN Clinical Manager to lead our clinical team with empathy, excellence, and a passion for quality end-of-life care. Job Summary: Responsible for the day-to-day management of patient care activities and supervision of clinical nursing staff. Is responsible for directing, supervising and ensuring patient care and services are delivered timely, responsibly and safely. Is responsible for the direct and indirect supervision of the nursing staff. Assists in quality improvement program. Job Qualifications: Education - Graduate of an accredited Diploma, Associate of Baccalaureate School of Nursing Licensure- Texas Driver's License Experience- Two years' experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred. Skills- Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of Hepatitis consent/declination. Specialized skills preferred in palliation and end-of-life. Transportation- Reliable transportation. Valid and current auto liability insurance. Essential Function and Responsibilities: Supervises patient care team in the delivery of patient/family care. Receives and assigns case referrals to personnel. Performs supervisory visits with RN, LVN, CNA. Assist Director of Clinical Services in ensuring that all patient/family services comply with Medicare COP's federal, state, and local regulatory requirements and with accepted professional standards. Reviews admission documentation for patient appropriateness. Participates in the development, implementation and evaluation of policies and procedures for patient/family services. Participates in the selection and evaluation of all direct patient/family care staff. In collaboration with the Quality and Education Coordinator, participates in the orientation and ongoing education of staff and volunteers. Coordinates the development, implementation and review of the interdisciplinary plan of care for each patient/family. Communicates as needed with all members of the team, the patient/family, the medical director, the administrator, and others involved in the care of the patient to facilitate optimal care of patient/family Maintains the clinical records, including nursing clinical notes and progress notes, for each team member under their supervision. Assists with the processing and maintenance of all patient related bills. Assure the 24-hour availability of nursing services to patient/families. Teaches patient and family members regarding nursing care skills, procedures, techniques and use of equipment. Participates in quality improvement reviews especially related to the evaluation of nursing services. Makes recommendations for improving and enhancing the overall functioning of agency. Manages the development of a discharge plan when a patient is discharged prior to death or transferred to another hospice. Responsible for time and attendance of all staff assigned. Participated in clinical administrative on-call rotation. Other duties as assigned Why Hospice of South Texas?
