Job summary

San Diego Home Health is hiring a Clinical Manager (RN) to lead the San Diego clinical team, oversee high-quality patient care, and ensure regulatory compliance. The role involves mentoring clinicians, coordinating interdisciplinary care, and maintaining strong relationships with patients and families.

Strong knowledge of Medicare CoP and home-health regulations is required, along with an active California RN license and nursing degree.

Qualifications

• Current California RN license in good standing.
• Graduate of an accredited school of nursing.
• Minimum of two years of clinical nursing experience within the past five years.
• Home-health leadership or supervisory experience preferred.
• Strong knowledge of Medicare CoP and home-health regulations.

Responsibilities

• Provide leadership, supervision, and support to a team of home-health clinicians.
• Coordinate delivery of high-quality patient care and promote positive outcomes.
• Ensure compliance with federal, state, local, Medicare, and Medi-Cal regulations.
• Facilitate interdisciplinary care conferences and clinical education.
• Review referrals and coordinate timely patient assessments and admissions.
• Assign patient care and oversee scheduling and on-call coverage.
• Monitor documentation and regulatory compliance; maintain patient records.
• Serve as a clinical resource, mentor, and preceptor for nursing staff.

We are seeking an experienced full-time Clinical Manager (RN) to lead our San Diego clinical team. This leadership role is responsible for overseeing the delivery of high-quality patient care, supporting and developing clinical staff, ensuring regulatory compliance, and partnering with interdisciplinary teams to achieve exceptional patient outcomes.

What You’ll Do

• Provide leadership, supervision, and support to a team of home-health clinicians.
• Coordinate the delivery of high-quality patient care while promoting positive patient outcomes.

What You’ll Bring

• Current California Registered Nurse (RN) license in good standing.
• Graduate of an accredited school of nursing.
• Minimum of two (2) years of clinical nursing experience within the past five (5) years.
• Previous home-health leadership or supervisory experience preferred.
• Strong knowledge of Medicare Conditions of Participation and home-health regulations.
• Excellent communication, organizational, and leadership skills.
• Ability to build positive relationships with patients, families, referral partners, and team members.
• Valid driver's license, reliable transportation, and current automobile insurance.
