Responsibilities

• Meet with prospective patients and families prior to discharge to explain services and prepare for transition.
• Conduct post-discharge follow-up with patients and families.
• Develop and maintain relationships with social workers, discharge planners, case managers, facilities and other referral sources.
• Conduct regular facility visits to support business development and referrals outreach.
• Coordinate referrals from initial inquiry through start of services.
• Review prospective patients' needs with clinical staff.
• Work with Director of Operations to coordinate transitions and follow-up.
• Identify new referral opportunities and strengthen facility relationships.
• Maintain documentation of patient/family interactions and referral activity.
• Represent Veridia Home Care professionally within facilities and community.

Position Summary

Veridia Home Care is seeking an outgoing, motivated, and compassionate Registered Nurse (RN) to join our team as an RN Clinical Liaison & Business Development Representative.

This position will serve as a bridge between skilled nursing facilities, patients and families, referral sources, and our home-care team. The ideal candidate will combine clinical knowledge with strong communication and relationship-building skills to support patient transitions from facility to home while helping develop and maintain referral relationships throughout the community.

Key Responsibilities

• Meet with prospective patients and families prior to discharge to introduce Veridia Home Care, explain available services, and help prepare for a smooth transition home.

• Conduct post-discharge follow-up with patients and families to ensure services are going well and identify any concerns or additional needs.

• Develop and maintain relationships with social workers, discharge planners, case managers, skilled nursing facilities, assisted living facilities, rehabilitation centers, and other referral sources.

• Conduct regular facility visits and support ongoing business development and referral outreach.

• Follow up on potential referrals and help coordinate the process from initial referral through the start of services.

• Review prospective patients from a clinical perspective and communicate relevant care needs to the appropriate agency staff.

• Work closely with the Director of Operations and clinical team to coordinate patient transitions and follow-up.

• Assist with identifying new referral opportunities and strengthening existing facility relationships.

• Maintain timely and accurate documentation of patient/family interactions, referral activity, and follow-up.

• Represent Veridia Home Care professionally within facilities and throughout the community.

• Provide nursing/clinical support as appropriate and within the individual's scope of practice.

Qualifications

• Current and unrestricted North Carolina RN license.

• Previous nursing experience; home care, skilled nursing, assisted living, rehabilitation, or long-term care experience preferred.

• Strong interpersonal, communication, and relationship-building skills.

• Comfortable speaking with patients, families, social workers, case managers, and facility leadership.

• Outgoing and comfortable with community outreach and business development.

• Strong organizational and follow-up skills.

• Ability to work independently and manage multiple referrals and relationships.

• Valid driver's license and reliable transportation.

• Current CPR/BLS certification preferred.
