Job description

Overview

Presbyterian is seeking an RN to provide direct patient care to an assigned group of patients in an Outpatient Clinic. Common responsibilities include preparing the patient for the visit, maintaining efficient patient flow, assisting the provider with treatments and procedures, medication administration, and support for clerical functions to include scheduling, referrals, authorization, and in-basket support with the EMR.

Work Schedule: Monday – Friday 8 hour days

Responsibilities
  • Responsible for walk-in and telephone triage.
  • Coordinates patient care through collaboration with the patient/family and healthcare team.
  • Provides patient care by assessing, planning, implementing, evaluating, and documenting individual patient needs, actions taken, and patient responses.
  • Responsible for case management based on PCP panels, in collaboration with network providers to include nurse visits, telephone management, etc.
  • Delegates or assigns aspects of patient care to healthcare team members commensurate with their validated competency to perform the task.
  • Documentation within scope of practice.
  • Performs other functions as required.
Qualifications
  • Registered Nurse (RN) - State of New Mexico or Compact State Nursing License required.
  • Associate degree in nursing.
  • BLS Certification required at time of hire.
  • Prefer 1 - 2 years ambulatory experience.
