Job summary

Crescent Hospice seeks a Registered Nurse with supervisory experience to lead an interdisciplinary team of RNs, Aides, Social Workers, and Chaplains as Clinical Director in Rock Hill. You’ll partner with the Program Director, Hospice Physician, and Community Liaisons to ensure compassionate, compliant hospice care.

The role emphasizes clinical leadership, staff development, and adherence to federal and state guidelines, with a Monday–Friday schedule and a strong commitment to patient and family

Qualifications

• Bachelor's degree in nursing or health care preferred.
• Registered Nurse with a current state license to practice as a professional nurse in the state the office is located.
• A minimum of 3 years experience as a nurse, including 1 year of direct patient care in hospice/home health/oncology, and 1 year of supervisory experience in hospice or home health care preferred.
• Demonstrated ability to manage all aspects of terminal care with crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
• Must be computer proficient, including EMR systems, and have CPR certification if required by state.

Responsibilities

• Coordinate and oversee a spectrum of patient services in alignment with hospice program policies.
• Guide and support an interdisciplinary team of clinicians.
• Conduct chart audits to ensure regulatory compliance and ethical care standards.
• Contribute to orientation and in-service training for nursing and clinical staff.
• Evaluate and uplift clinical staff performance during probation and annually, providing coaching as needed.
• Lead interdisciplinary team meetings to refine patient/family Care Plans and foster collaboration.

Responsibilities

• Champion Comprehensive Patient Support: Coordinate and oversee a spectrum of patient services, ensuring alignment with established hospice program policies.
• Empowering Teams: Provide guidance and support to an amazing team of clinicians, fostering continuous enhancement of hospice care services provided.
• Quality Assurance: Conduct meticulous chart audits, overseeing medical record upkeep for compliance with regulations, certification standards, and ethical care benchmarks.
• Training Excellence: Participate in crafting orientation and in-service training for nursing & clinical staff, nurturing their growth across all proficiency levels.
• Performance Advancement: Evaluate and uplift clinical staff performance during probation and annually, offering valuable teaching and counseling to enhance the delivery of services from our clinicians.
• Collaborative Leadership: Lead interdisciplinary team gatherings to refine patient/family Care Plans, encouraging the exchange of insights, resolution of challenges, and nurturing professional camaraderie.

Qualifications

• A minimum of 3 years experience as a nurse, including 1 year of full time experience in providing direct patient care in a hospice, home health or oncology setting, and 1 year of supervisory experience in hospice or home health care preferred.
• Demonstrated ability to manage all aspects of terminal care with expertise in crisis intervention, pain and symptom control, clinical bedside nursing, and family counseling.
• Demonstrated ability to execute a supportive, sensitive approach to staff and patient/family needs.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$94,000 - $100,000 per year (Average Salary Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
