Responsibilities

What You’ll Do:

Review OASIS documentation for admission, resumption, recertification, and discharge assessments for Medicare, Medicaid, and HMO cases.

Ensure all documentation aligns with the Plan of Care and medication profile.

Collaborate with field clinicians to validate documentation accuracy and completeness.

Qualifications

• RN with Bachelor’s degree inNursing
• OASIS Certification required
• 3-5 years of home careexperience
• Strong knowledge of OASIS andMedicare/Medicaid guidelines
• Strong computer skills
• Excellent verbal and writtencommunication skills
• Excellent interpersonal skills
