Haven Hospice in Gainesville, FL is seeking a compassionate Clinical Community Liaison (RN) to promote hospice care among hospitals, nursing homes, and community providers. You will identify patients who may benefit, coordinate care with admitting teams, and guide families through transitions to hospice services.

The ideal candidate is an RN with licensure in Florida, 3+ years nursing experience (hospice preferred), and strong compliance and analytical skills.

Qualifications

• RN with current licensure in the assigned state.
• BSN preferred.
• Three years’ nursing experience, hospice experience preferred.
• Two years of experience in compliance, internal audit, basic law or paralegal strongly preferred.
• Excellent understanding of compliance, Medicare Conditions of Participation, and medical documentation standards.
• Ability to manage highly confidential information.
• Analytical skills with ability to define problems, collect data, establish facts, and draw valid conclusions.
• Meticulous attention to detail.
• Self-director and ability to work with minimal supervision.

Responsabilités

Qualifications

• Registered Nurse with current licensure to practice in assigned state(s)
• Bachelor’s degree in Nursing from an accredited program by the National League for Nursing preferred
• Three years’ experience in clinical nursing, hospice experience preferred
• Two years of experience in compliance, internal audit, basic law or paralegal strongly preferred
• Excellent understanding of compliance, Medicare Conditions of Participation, and understanding of medical documentation standards
