RN Clinical Care Coordinator, Long Term Care Facility

Join Oswego Health as an RN Clinical Care Coordinator at the Long Term Care Facility. This role involves promoting coordination of care for residents, monitoring, documenting, assessing, and evaluating interdisciplinary care from admission to discharge. Additionally, the coordinator assists in overseeing daily unit functions alongside the Unit Manager.

Responsibilities include:
  • Reviewing reports, making rounds, and documenting clinical findings.
  • Collaborating with the healthcare team and families.
  • Managing admission processes and ensuring proper documentation and care planning.
  • Overseeing care delivery, updating care plans, and ensuring smooth discharge processes.
  • Participating in educational and supervisory duties as needed.
Qualifications:
  • Current NYS RN license.
  • Experience in care coordination or management preferred.
  • Supervisory experience is a plus.
Skills:
  • Effective communication and problem-solving skills.
  • Organizational and leadership abilities.
  • Knowledge of aging, resident rights, infection control, and safety regulations.
Work Conditions:
  • Frequent walking, standing, and physical activity.
