Job summary

AdventHealth in Sebring, FL seeks a registered nurse to join our care team. You will assess patients’ needs, develop care plans with an interdisciplinary team, and provide patient and family education.

You will coordinate care for high-risk patients, participate in group visits, and support transitions of care while maintaining accurate documentation in the electronic health record. RN licensure and BLS are required.

Qualifications

• Associate degree in nursing required; RN license required.
• Preferred Bachelor’s degree in nursing.
• Experience with patient education and care planning preferred.

Responsibilities

• Assess patients’ physical, psychosocial, cultural, and spiritual needs.
• Develop and implement patient care plans with inter-disciplinary team.
• Educate patients and families and evaluate teaching effectiveness.
• Coordinate high-risk patient group visits and transitions of care.
• Maintain accurate nursing documentation in the EHR.

Job Description:

• Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards. Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocates for and protects the health, safety, and rights of each patient.
• Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Other duties as assigned.
• Coordinates and implements group visits for identified high-risk patient populations.
• Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
