Windsor Nursing and Rehabilitation Center of Weslaco in Weslaco, TX is seeking an RN Charge Nurse to support high-quality resident care. You will oversee shift protocols, monitor conditions, and ensure care plans are followed.

The ideal candidate holds an active Texas RN license, completed an accredited RN program, and has experience in a skilled nursing facility. Strong assessment, communication, and leadership skills are essential to coordinate physicians, staff, and residents.

Qualifications

• Has completed an RN program at an accredited vocational school, college, or university.
• Must hold an active Registered Nurse license in Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Ensure care plans are implemented as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in resident condition and take action per policy.
• Oversee and monitor aides.
• Initiate and complete incident and accident reports per policy.
• Discharge residents per facility policy.
• Complete Nursing Assessments prior to physician calls.
• Communicate status updates to residents' contacts when asked or if there is a change in condition.
• Assist with other special projects as assigned by the Director of Nurses or the Administrator.
