Job summary

The Avenues Transitional Care Center in San Francisco, CA, is seeking an experienced RN to oversee and support CNAs, ensuring timely, high-quality patient care and accurate clinical documentation.

You will interpret physician orders, monitor conditions, administer medications, and collaborate with the DON and leadership to maintain a compassionate, compliant care environment. New grads with active RN licenses are encouraged to apply.

Qualifications

• Current Registered Nurse (RN) license valid in the same state as the facility.
• 1-2 years of experience in a Nursing or similar Facility preferred
• Customer service and emotional maturity – ability to remain calm and kind in all situations
• Outstanding communication (verbal and written), and organizational skills
• New graduates with active RN license are welcome to apply

Responsibilities

• Observe, document and report on symptoms and any change in conditions of assigned patients/residents
• Take and record temperatures, respiration and pulse
• Administer medications and treatments as prescribed, recording results/outcomes
• Set up treatment trays, clean and prepare equipment as needed and directed by DON
• Maintain accurate, understandable patient care records and other clinical documentation
• Identify process, or other facility issues, offering solutions for improvements to DON or Administrator in a professional collaborative manner
• Lead by example in supporting a kind, collaborative and supportive work environment
