Position Overview:

As a Certified Diabetes Educator (CDE) at Aizer Health, you will play a vital role in empowering patients to take control of their gestational diabetes management. You will educate and support individuals with gestational diabetes, helping them understand the disease, manage their health, and improve their quality of life. Through education, counseling, and ongoing support, you will enable patients to make informed decisions that promote long-term health and wellness.

Position Overview

- Patient Education: Provide comprehensive diabetes education and counseling to pregnant patients for managing gestational diabetes, focusing on nutrition, medication management, insulin administration, and lifestyle changes.
- Care Coordination: Collaborate with physicians, nurses, and other healthcare professionals to develop personalized care plans tailored to each patient’s needs, ensuring a multidisciplinary approach to diabetes management.
- Chronic Disease Management: Guide patients through diabetes self-management techniques, including blood glucose monitoring, carbohydrate counting, and exercise plans, to help them achieve better control over their health.
- Support and Counseling: Offer ongoing support and counseling to patients, addressing any concerns, helping them navigate the emotional aspects of diabetes, and empowering them to maintain healthy habits.
- Community Engagement: Participate in community outreach programs, workshops, and group education sessions to raise awareness about diabetes prevention and management within the community.
- Documentation and Reporting: Accurately document patient progress, interventions, and outcomes in the electronic medical record system, and communicate effectively with the broader care team to ensure continuity of care.
- Follow-up Care: Conduct follow-ups to monitor patients’ progress, adjust care plans as necessary, and ensure they remain compliant with medications, lifestyle changes, and recommended check-ups.

Schedule:

Monday- Friday 9 am - 5pm, 1 closing shift from 12pm - 8pm

Minimum Qualifications:

- Certified Diabetes Care and Education Specialist (CDCES) or eligibility to sit for the exam.
- Registered Nurse (RN).

- Experience counseling for and managing gestational diabetes within an MFM department and/or an Endocrine practice.

- Strong knowledge of diabetes self-management practices and patient education techniques.
- Excellent communication and interpersonal skills, with a passion for patient-centered care.
- Ability to work effectively in a collaborative, multidisciplinary environment.

Preferred Qualifications:

- 3+ years of experience as a Diabetes Educator.
- Experience with Electronic Medical Records (EMR) systems, such as ECW or similar.
- Bilingual skills (Spanish or Yiddish) are a plus.
