Job summary

Continuum Hospice in California is seeking a full-time Hospice RN Case Manager (RNCM) to join our compassionate nursing team, delivering hands-on hospice care, coordinating services, and guiding patients and families through their journey.

The position offers Monday–Friday 8 am–5 pm in the San Jose territory with a $64–$66 hourly pay range, plus a robust benefits package, tuition reimbursement, PTO, and a career path program to support growth.

Qualifications

• Hold current unencumbered RN license.
• BSN or NLN-accredited nursing program; OR at least 1 year of nursing experience in last 3 years.
• Bilingual in English and Spanish preferred, not required.
• Hospice/home health experience preferred but not required.
• Computer proficient with typing and EMR experience.
• CPR certification current; valid driver's license and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise to ensure exceptional service.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Deliver hands-on hospice nursing and coordinate care for patients.
• Provide nursing care rooted in proven principles to ease pain and support comfort.
• Educate and advocate for patients and families to create personalized care plans.
• Participate in call duties as part of team responsibilities.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
  • One (1) year experience as a professional nurse within the last three (3) years.
• Bilingual in English and Spanish perferred, not required
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$64.00 - $66.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
