Job summary

Community Care, Inc. Home Health seeks a compassionate Registered Nurse (RN) to provide skilled home health care in New Stanton, PA. Weekend schedule (Saturday & Sunday) with hours varying, covering Westmoreland and Fayette County.

You will build meaningful connections, administer medications, and support patients in their homes with a patient-centered approach. Join a supportive team, receive weekly pay, orientation, and opportunities for growth while delivering high-quality care in the home

Qualifications

• Current RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience (Home Health preferred).
• CPR certification is required.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments).
• Develop and implement individualized care plans.
• Educate patients and caregivers on managing conditions and medications.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Document visits and patient progress accurately and timely.
