Job summary

DirectShifts is seeking an RN Case Manager to support telehealth-enabled patient care at our Royal Oak, MI office. The role combines in-person and virtual coordination to deliver individualized care plans and continuous follow-up.

Responsibilities include assessments, patient education, and collaboration with clinicians using an EHR system. Offers both full-time and part-time schedules with onsite work in Royal Oak.

Qualifications

• Active, unrestricted Michigan RN license.
• Current BLS certification.
• Minimum of 2 years of clinical nursing experience.
• Strong clinical assessment and communication skills.
• Experience with EHR systems and telehealth platforms.

Job description

We are currently hiring RN Case Managers for our Royal Oak, MI, office and are open to both full-time and part-time candidates.

Position Details:

• Job Title: RN Case Manager (Telehealth)
• Location: Royal Oak, MI (Onsite)
• Employment Type: Full-Time or Part-Time
• Full-Time Schedule: Monday–Friday, 8:00 AM–4:00 PM
• Part-Time Schedule: Flexible based on availability and operational needs.
• Work Environment: All patient care is provided virtually from our Royal Oak office. This is not a remote or travel nursing position.

Key Responsibilities:

• Conduct patient assessments and develop individualized care plans.
• Provide virtual patient education, follow-ups, and chronic care management.
• Monitor patient progress and coordinate care with healthcare providers.
• Document patient interactions and care plans accurately in our EHR system.
• Identify and elevate patient concerns as appropriate.
• Collaborate with our interdisciplinary clinical team to ensure continuity of care.

Required Qualifications:

• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work with electronic health records (EHR) and telehealth platforms.
• Strong patient education and care coordination skills.

Preferred Qualifications:

• Previous experience in case management, chronic care management, or telehealth.
• Certified Case Manager (CCM) certification.
• Experience with remote patient monitoring (RPM).
