Qualifications

  • At least 2 years of experience in case management or a related role.

The RN Case Manager Team Lead supports effective discharge planning and utilization review activities. S/he will manage performance of Clinical Resource Coordinator / Case Manager staff and provide them with continuing educational programs/training; collaborate with the Director to develop efficiency within the department. S/he are also involved in staff scheduling and ensuring all areas of the hospital have adequate case management coverage.

Performance Management - Quality Reviews: Complete quality reviews for Clinical Resource Management staff to ensure appropriate performance and documentation. Provides constructive feedback of quality reviews, including an explanation of the score provided, areas for improvement and comments about positive performance. Communicates significant changes in individual quality review scores to the CRM Associate.

  • Educational Coordinator: Contributes to the design and development of professional standards of practice, hospital and department goals, workflow design, and department operations and documentation systems. Implements processes for collecting customer service/patient satisfaction comments and disseminating feedback to the Clinical Resource Management & Social Work Department. Leads development and implementation of department’s annual educational program. Assures thorough orientation is developed for new hires.
  • Vacation/Unexpected Absences Manager: Receives and organizes vacation requests for CRM staff and determines which requests will be granted. Informs staff of approved or declined requests. Informs staff of who and when they will be responsible for vacation coverage and facilitates arrangement during coverage dates (observing work volume to ensure it does not increase above expected range, providing pagers to covering staff, etc.). Collaborates with CRM Associate Director to determine vacation requests during high-peak request times (holidays, summer) to ensure fairness and appropriate coverage within pods. Reassigns work load within pods when unexpected.
  • Discharge Planning: Completes timely and detailed assessment of patient and family social, emotional and/or discharge needs and health care benefits. Develops and facilitates specific plan to address problems and/or concerns identified by patients, families and health care team. Identifies and consistently documents delays; elevates issues appropriately. Effectively communicates discharge plan with patients, families and health care team. Documents ongoing assessments and plan of care in patient record (Canopy). Successfully assists patient in moving to the next level of care with transfers to home, agencies, and facilities and with referral to appropriate community resources. Participates and drives accountability in daily interdisciplinary meetings.
  • New Technology / Computer Systems: Keeps current with new systems (Penn Chart, MedView, ChartOne, Ports, etc.). Incorporates technology into daily assignment (Outlook, Canopy, CERME, ECIN, I drive, Internet).
  • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures
  • Other duties as assigned to support the unit, department, entity, and health system organization

Requirements:

BSN and PA RN

2 plus years of experience in case management
