UCLA Health in Los Angeles is seeking an experienced Advanced Practice Nurse to assess and coordinate care for a diverse patient population. You will collaborate with a multidisciplinary team and patients' families to design and implement individualized care plans using evidence-based risk stratification tools, ensuring safe, high-quality, cost-effective care across transitions.

Using your advanced practice nursing skills, you will be responsible for assessing and coordinating care for a diverse group of patients. You will collaborate and consult with a multi-disciplinary healthcare team as well as with patients and their families to ensure safe and effective coordination of care. This involves developing and implementing individualized care plans utilizing evidence-based tools for risk stratification to ensure delivery of safe, high quality, efficient, and cost-effective care. You will also perform utilization review while assuring the delivery of concurrent and post-hospital care. We’re also looking to you to help drive performance improvement efforts.
Salary Range: $65.53 - $84.75Hourly
We’re seeking a self-directed, creative problem solver with a:
General Medicine, ED, pediatrics, and surgery/transplantexperience is preferred.
