Qualifications:
  • Must have prior Case Management experience and 3 years of acute care experience.
Locations:

Float coverage at our local St. Louis ministries: St. Joseph St. Charles, St. Joseph Lake St. Louis, St. Clare - Fenton, St. Mary's St. Louis, and SLU Hospital St. Louis.

Job Summary:

Assesses, develops, plans, implements, and evaluates patients' discharge planning needs to the next level of care in collaboration with the interdisciplinary team.

Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
  • Ensures compliance with Condition of Participation (CoP) discharge planning.
  • Identifies and escalates gaps/barriers related to inpatient and observation length of stay.
  • Assesses patients at high risk for readmission, identifies root causes, and develops plans in collaboration with the multidisciplinary team.
  • Participates with other team members regarding opportunities for improvement in standard work.
  • May be responsible for utilization management, ED case management, admission process, and acting as a liaison with post-acute providers.
  • Applies evidence-based practice and scientific knowledge in nursing, ensuring care is based on patient’s age-specific and clinical needs.
  • Demonstrates professional nursing standards and uses the ANA Code of Ethics for Nurses.
  • Works safely and performs other duties as assigned.
Education:
  • Graduate of an accredited school of nursing or equivalent licensing.
Experience:
  • Three years of acute care experience.
Physical Requirements:
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
  • Frequent sitting, standing, walking, reaching, and repetitive movements.
  • Use of vision, hearing, speech, keyboard, and occasional physical movements.
Licenses and Certifications:
  • Registered Nurse (RN) license in Illinois, Missouri, Oklahoma, or Wisconsin, as applicable, issued by the respective licensing authority.
