Job Summary:

The Registered Nurse Case Manager (RN Case Manager) provides direct patient care to hospice patients and supportive care to their families. Manages an interdisciplinary group (IDG) including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality. Responsible for case managing all assigned patient clinical records.

Schedule: Full-time, Monday-Friday 8:30am-5:00pm

Coverage Area: South Territory - Denver, Aurora, Green Valley Ranch

Major Job Functions:
  • Promote and practice the Sage Family of Companies philosophy and mission of providing compassionate, comprehensive, and qualified care to people with a life-limiting illness.
  • Ensure the highest quality of care is provided and documented per the Plan of Care (POC).
  • Perform initial and ongoing assessments of patients.
  • Develop and implement the POC in collaboration with the physician and IDG.
  • Educate patients, families, and caregivers on needs identified in the POC, including medication administration, dressing changes, oxygen use, hospice philosophy, and care of the terminally ill.
  • Work closely with the IDG to provide physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
  • Maintain primary control and management of each patient, acting as liaison between physician, patient, family, and hospice team.
  • Report any changes in patient condition to the attending physician and hospice physician.
  • Maintain communication with family and care staff, and keep accurate medical records.
  • Review and follow-up on patient records to ensure quality, standards, and compliance.
  • Practice infection control and standard precautions.
  • Conduct supervisory visits with team members monthly and monitor staff scheduling.
  • Maintain knowledge of pain and symptom management in the dying process.
  • Participate in quality assessment and performance improvement programs.
  • Attend staff meetings and participate in on-call schedules if required.
  • Promote Sage Hospice and Palliative Care core values and complete annual training.
  • Perform other duties as assigned.
Required Education/Experience:
  • Registered Nurse with a current license in the practicing state.
  • At least 1 year of experience in oncology, long-term care, hospice, or home health.
  • Current CPR certification.
  • Reliable transportation, proof of insurance, and a valid driver’s license.
