Job summary

Affinity Hospice is seeking a dedicated registered nurse/case manager to plan, organize, and provide hospice care in patients' homes and communities in the Marietta area. The role emphasizes community health education and coordination with an interdisciplinary team.

You will manage patient plans of care, ensure quality, comply with OSHA infection control, participate in on-call service, supervise aides and LPNs, and maintain thorough documentation.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with insured vehicle and good working order.
• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.
• Able to lift, position and/or transfer patients; able to lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect both staff and patient.
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals.
