RVNAhealth in Ridgefield, CT is seeking a Full‑Time RN Case Manager to provide direct patient care and case management in the home setting or facilities. You will oversee admissions, plan of care, and coordinate services with physicians to meet patient needs.

We are currently seeking a RN Case Manager in Ridgefield, CT. This includes admission of patients for service and determining and developing a plan of care.
This position will cover the town of Ridgefield, CT and surrounding local area.

Our target pay range for this position is $95,000 to $103,000 per year. This pay range represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
